_____ I agree to pay nine (9) tuition payments. The first tuition payment is due on the first day of May 2012 or as soon as I register my child. The second payment is due on the first day of school, September 2012. Each payment thereafter is due on the first day of the month.

_____ If I withdraw my child without 30 days notice, I owe next month’s tuition.

_____ Registration Application fee ($75) is NON-refundable. Make checks payable to MVWS.

_____ Deposit only refundable if more than 30 days notice given prior to first day of school.
